What is palliative care at home?
Palliative care at home is specialised medical care for people living with a serious illness. Our palliative care service focuses on relieving and preventing the suffering of patients, to improve the quality of life for patients and their families, no matter the diagnosis or stage of the disease. Palliative care at home is provided by a team of doctors, nurses and other specially trained people. We work together to provide you the support you need, whilst enabling you to maintain the place you feel most comfortable, your home.
How do I arrange palliative care and what packages are available to me?
Firstpoint Homecare has a wealth of experience supporting those living with chronic conditions and we will put a support plan in place that works for you. We know the importance of personalised care for everyone and never use a “one size fits all” approach. Whether you want us for one hour per week, one hour per day, overnight, you simply tell us your requirements and we will meet them.
If you would like to arrange a no obligation home visit, need more information on our care services, or would like general advice and guidance on funding, integrated care and hospital discharges, please get in touch today.
